I have no idea how to add a package on DA, so this is the best I can do for you:

Background: Mac OSX Leopard
Theme: Clear Looks

I used Icon Packager for the Icons, you'll have to download it, if you don't already use it. Dowload Icon Packager
Icon Theme: Denim Plastic

NOTE:

Theme is only for a Windows XP System.
If you have Linux, Windows 7, Windows Vista, etc - It won't work correctly.
